Getting Started at ONPT

Referrals

Referrals are required for most insurances. We accept referrals from Physicians (including but not limited to MDs, DOs, NDs), Physician Assistants, Nurse Practitioners, Dentists, and Chiropractors.

Please check your insurance to see if a referral is required for insurance coverage. 

Oregon State Law allows for practice without referral. However, many insurances do require referrals.

We also accept cash-based patients without referral.

Scheduling with us

Most, but not all, insurance will require a referral from your physician before beginning physical therapy.

Once you have secured a referral for physical therapy, you may call the clinic and begin the intake process.

Our staff will guide and educate you through the steps at that time.​

New Patients

For caregivers

Caregivers, Guardians and Family Members are encouraged to be present for continuity of care. Please remember to include an Emergency Contact Number in the patients’ paperwork.

For Minors

We appreciate if a parent or guardian is present during treatment with minors.

Additional information for new and existing patients

  • We appreciate you giving us the opportunity to treat you. Please let us know as soon as you can of any scheduled surgeries so we can schedule the pre-operative rehabilitation in the appropriate timeframe.

  • While we do understand that unforeseen events occur, we ask for 24-48 hours notice to cancel or reschedule appointments. With advanced notice, we are able to accommodate patient needs. If not given a minimum 24-hour notice, patients will be charged a $50.00 fee. This amount must be paid at the next appointment.

  • A “No Show” is a missed appointment without communication in advance. (Please see our Cancellation Policy). No Shows inconvenience four people: 1.) you 2.) Physical Therapist 3.) Front Office Staff, 4.) patient on waitlist.

    Failure to arrive at the time of appointment two consecutive times will be recorded in the patient’s chart as a No Show, which will result in all future appointments being canceled. If the patient fails to show up for their initial Evaluation Appointment all further scheduled appointments will be canceled.
